I had this same issue. I'm using virualbox running Windows 10 on a Mac. I wasn't able to install the virtualbox extensions at first so I was doing everything with USB1 emulation, and it wouldn't connect.

The device is not available as a USB device in Windows It's not appearing as a faulty or unknown device in Device Manager; it is completely missing! That suggests something quite low level. EDIT: Got it working. Either I needed to re-run "Flash custom kernel" or I had never actually done that. I suspect the instructions I was following online were missing this step.
Windows 7 x64 Enterprise. First usb slot didn't work. Also be sure to follow the steps to have it turned off and hold reset then power. Atleast on my windows it will make a noise when its picked up.
